What We Do
We value our case managers and the relationship we have with you. In working together to further your client’s recovery or rehabilitation we want to make the process easy for you. Please let us know how we can help you.
Our prescriptions are valid for 3-6 months. We request a new Rx 30 days prior to expiration for program continuation. Once we receive the signed Rx from the Doctor, we will send you a copy via fax.
Each session will have documentation showing the progress of your client and how that relates to the goals for their care plan. Your clients’ monthly Progress Notes will be sent to you via fax once it is completed by their Physical Therapist or Occupational Therapist
Should your client need orthotics, we will work with you and the client to ensure the proper orthotic is in place.
We’re happy to accommodate DME appointments such as wheelchair evaluations or service appointments for clients during our lunch hour from 12 noon to 1 pm. We do not allow DME appointments to be scheduled at our facility during a client’s session time.
We are happy to fax or email you if your client cancels or misses a session. Please let us know if this is a service you would like.
We are available for scheduled team meetings with you, your client, our Physical therapist, and others involved in your clients’ recovery. We have access to conference rooms to hold these private meetings at Walk The Line with Zoom in options available.
We would like to extend an invitation for you to tour our facility. You are welcome to stop in at any time during business hours or contact us to set up a tour for you and your client.
